As of February 2017, we offer online video appointments for existing patients with commercial insurance. Co-payment and co-insurance is due at the time of the service. This service is also available to existing cash pay patients. All participants must have their own computer technology with Google Chrome (including working audio and video) to use in a quiet, private environment in order to participate. We are now located in the Willits Care Center, the new gray and orange building located directly in front of the Whole Foods Parking Lot in Willits Town Center. Park on the other side of that building (OPPOSITE to the Whole Foods parking lot). Enter at the door marked 350 and come up the elevator or stairs to the third floor. Turn right, and we are the first door on the left.

Insurance and Payment Information
Our practice is NOT in a physician network with provider-based billing. We are not owned by any hospital. What does this mean for you? Unlike those kinds of practices, you will only receive one bill from us. With provider-based billing from practices where the doctors are owned by the hospital, patients are charged for both physician (or non-physician provider) professional services (Part B) and the facility fee (Part A). That model generates two separate charges, two co-payments, and typically two bills, all for one episode of care. Again, our practice is a stand alone clinic where you will not be charged a facility fee, just for Dr. Allen's associations with the two hospitals where she is on staff. This is a huge benefit and cost savings for you as a patient in our clinic.
Dr. Allen is a provider for Medicare and most insurance plans. We provide insurance billing; however, if you have not met your deductible for the year, payment will likely be due on the day of your appointment. Please be prepared to pay on the day of service. Anything not covered by insurance will be your responsibility. Your insurance company may also require you to pay a co-payment which should be paid at the time of service. Any and all additional co-insurance, deductibles, and/or other financial obligations as determined by your insurance carrier will be billed to you upon determination, and you are expected to pay these upon receipt. If you do not have insurance, ask about our cash discount policy.
If you have any questions regarding which insurance plans we accept or any other billing concerns, please call our billing service, Professional Medical Services at 520-296-1206. Please also notify us of any changes in your insurance coverage.
Our office is generally open Monday through Friday from 8:00 a.m. to 4:00 p.m. After regular hours during the week, on weekends, and on holidays, the answering service will take messages and relay them to Dr. Allen. This service should be utilized only for urgent problems that need resolution prior to the next scheduled work day.
If you need renewals of your current prescriptions, please call the office during regular working hours at least one week in advance. You may also have your pharmacy fax a request to 970-422-7123.
